44 year old professional San Diego woman in the aesthetic industry had previously undergone rhinoplasty elsewhere and was disappointed with the functional and aesthetic outcome. She had bilateral nasal airway obstruction from collapse of both nostrils on inspiration due to bilateral external valve collapse. She disliked the appearance and shape of her nose as well. Her nasal tip was pinched and twisted and there was a residual hump along the bridge of her nose. At surgery, the external valve collapse was corrected with bilateral alar batten onlay grafts, columellar strut, and an extended tip graft to correct the pinched and twisted nasal tip. A dorsal hump was removed and the nose was narrowed with osteotomies. After surgery she was able to breathe normally through both nostrils and was thrilled with the aesthetic result which appeared quite natural.
